Undifferentiated pleomorphic sarcoma is an exceedingly rare and aggressive primary cardiac tumor, and left atrial undifferentiated pleomorphic sarcoma is an even rarer subtype, occurring in 1% of cases. Prognosis is poor, with survival after diagnosis often <1 year. In this case, the patient presented with the nonspecific symptoms of angina and dyspnea on exertion. Cardiac MRI showed multiple masses in the left atrium that pathology identified as high-grade undifferentiated pleomorphic sarcoma. Following surgical resection and chemotherapy, the patient survived >2 years.
